Required Documents for Canadian Citizenship Certificate Application:

(Please be advised that it takes at least 9-13 months for Nova Scotia to process one Citizenship Application. For information of applying for/renewing a Canadian Citizenship Certificate, please Get proof of citizenship.   • Applying for Canadian Citizenship Certificate for a child born abroad to Canadian parent(s)
    1. One (1) set of Canadian Citizenship Certificate application form. (Please make a photocopy for your records.)
    2. Two (2) identical photographs of the correct size as per the citizenship application form.
    3. The child's ORIGINAL English official birth certificate which shows the child's personal birth details and indicates the full names of the natural parents.  If the English birth certificate of the child cannot meet the requirements stated above, and the child is registered with the Taiwan household registry, we would also accept the Household Registration record issued by the Taiwan Household Registry Office. If the document is not issued in English, please have the document translated and have the translation certified at the local courts.
    4. Both parents' passport and the Canadian parent(s)' proof of Canadian citizenship ie: a Canadian Birth Certificate or a Canadian Citizenship Certificate. We must see the ORIGINAL documents, photocopies will not be accepted.
    5. If available, the ORIGINAL marriage certificate of the child's parents.

      * It is recommended that the child’s citizenship is submitted before the child reaches the age of two. If your child is over two years old, in addition to the above mentioned documents, please also provide:

    6. the child’s Household Registration Transcript (you can apply for the transcript from the household Registration Office where the child is registered).  Note that the child’s birth registration date must be clearly indicated on the Household Registration Transcript.
    7.  self-statement with the reason why you have not applied for the child’s citizenship certificate before the child turned two years old.  The self-statement must include the date and place where the child’s birth was registered.

 

Reapplying for a Canadian Citizenship Certificate due to loss

  1. One (1) set of Canadian Citizenship Certificate application form CIT0001
  2. Two (2) identical photographs of the correct size as per the citizenship application form.
  3. Solemn Declaration concerning a citizenship certificate that was lost, stolen, destroyed or never received CIT0457 (http://www.cic.gc.ca/english/pdf/kits/citizen/CIT0457E.pdf).
  4. Current Canadian Passport
  5. Large commemorative certificate
  6. Two valid Canadian IDs bearing name, gender, date of birth, photo and signature.  Please also submit the Taiwan passport and the Taiwan Household Registration Record in English if you are also holding a Taiwanese citizenship.

      

Reapplying for a Canadian Citizenship Certificate for replacement of an old, damaged certificate, or replacement due to name change

  1. One (1) set of Canadian Citizenship Certificate application form CIT0001
  2. Two (2) identical photographs of the correct size as per the citizenship application form.
  3. Statement explaining the circumstances of the loss CIT0457 (http://www.cic.gc.ca/english/pdf/kits/citizen/CIT0457E.pdf).
  4. Current Canadian Passport
  5. The original citizenship certificate and the large commemorative certificate
  6. Two Canadian IDs bearing name ,gender, date of birth, photo and signature.  Please also submit the Taiwan passport and the Taiwan Household Registration Record in English if you are also holding a Taiwanese citizenship.       

Applicant (the Canadian parent or the non-Canadian parent) must come in person at the time of lodgement due to security reasons. Please note that the child does not have to come.

Citizenship Certificate Pick-up and Mail-back service

Please note that, due to security reasons, every application needs to be made in person. However, for citizenship certificate collection, applicants may pick it up in person, or may request a mail-back service. Applicants should make their decision clear when submitting the application. For in-person pick up, please submit the pick-up receipt and a piece of English ID with photo and signature. To pick up a child's citizenship application, in addition to the mentioned two documents, please also bring the child's birth certificate or the English Household Registration Record for security reasons. Applicants who wish to have the citizenship certificate mailed back should provide our office with a self-addressed (address in Chinese is suggested) and NTD$60 self-stamped A4 size envelope when submitting the application.
